Program Data Specialist

Reach UR Life Program, Main Offices

 

Develops internal methods of data collection and reporting. Work collaboratively with program staff and contracted project evaluator to develop evaluation components for project. Collects all survey/evaluation materials at the completion of training, event, or activity to ensure materials reach the external programmatic evaluators. Identify and achieve consensus on data elements to be captured within database with project partners and evaluators. Develop method of reporting data to project partners, key stakeholders, community members, and local and national government. Attends meetings with Coconino County Pediatric Injury Prevention Task Force, Flagstaff Medical Center and Coconino County Coroner’s Office to determine targeted dates for data collection.

Human Resources Technician

Main Offices

 

Assists with the recruitment processing of scheduling and sending out notices to candidates. Works closely with payroll department and other department with the organization. Assists with benefits processing (i.e. orientation, enrollment, changes, terminations, etc.). Assists in maintains and update personnel files including medical files and in a timely manner.  Assists in orientation of staff on HR processes and policy related information. Completes and monitors employees’ compliance with employment requirements, including fingerprint clearance cards, certifications, background clearances, and trainings. Assists in prepares Personnel Action Forms (PAF) and maintains accurate employee records, including those related to hiring, promotions, terminations, position transfers, performance evaluations, performance issues, benefits, and other related changes as needed.

Project Assistant

Lasting Indigenous Family Enrichment (LIFE) Program

 

Provides group classes and one-on-one instruction using a variety of fitness and exercise techniques. Assist in connecting native history and cultural teachings to health and wellness lifestyle

Assist to provide seasonal cultural practices that support health and wellness. Works closely with the Health Promotion Team, medical providers and partners to provide patients/participants with comprehensive care to help individuals and groups with their health goals. Tracks program participant progress (A1C, lipid panel, weight loss, exercise goals) and other indicators to evaluate program success. Ensures proper electronic health record documentation, confirms charts are securely filed and in a timely manner.

Psychotherapist

Family Health Center

 

Conduct clinical mental health and bio-psychosocial assessment to clients presenting with mental health concerns, adjustment disorders, and comorbid substance abuse disorders. Diagnose mental health and comorbid substance abuse disorders in accordance with the Diagnostic and Statistical Manual of Mental Disorders (current edition). Collaborate with clients to devise and enact a client-directed treatment plan for mental health treatment. Provide individual, couples, family, and group psychotherapy in accordance with an individual’s treatment plan and presenting issues/concerns. This treatment will be provided at an interval appropriate to the acuity of their mental health condition. Maintain the client’s charts in a timely manner, and document all clinical contacts in the electronic health record.

Fitness Coach

Wellness Center

 

Conducts fitness assessments on participants using the American College of Sports Medicine guidelines. Evaluates individuals’ abilities, needs, and physical conditions, and develops suitable training programs to meet any special requirements. Primary focus is on pre-diabetic and diabetic clients. Works closely with the Health Promotion Team, medical providers and partners to provide patients with comprehensive care to achieve individual and group goals to improve: A1C, cholesterol profiles, weight loss, exercise goals and other indicators essential for tracking program success.

Director of Operations

Family Health Center

 

Designs, develops, improves, and implements Clinic Programs that are motivational, innovative, enjoyable, and effective in bringing about improved health status for program participants, their family members, and the community at-large. Ensures the Health Clinic and Health Promotion Program goals and objectives are met; oversees and monitors multiple budgets; generates grant reports; collects and assesses quantitative data; evaluates program effectiveness utilizing best case practice, empirical measures at baseline and at various predetermined time points for outcome measures. Develops program protocol and related procedures for facilitating collaborative processes among providers and other staff within the Family Health Center.
