Family Health Center

An outpatient, primary care clinic that offers family medicine in a caring and culturally sensitive environment.

The mission of the NACA Family Health Center is to promote and improve the health of individuals, families and the Flagstaff community through prevention and comprehensive treatment of illnesses or conditions.

The NACA Family Health Center (FHC), which opened in 1991, is one of 34 Urban Indian Health Centers in the country. The NACA FHC is a member of the National Council of Urban Indian Health and of the Arizona Association of Community Health Centers. The FHC is licensed through the Arizona Department of Health Services as a primary health care clinic.

Medical Services: Medical services are provided by board-certified physicians, registered nurses, and health educator. All staff have had extensive experience in working with Native Americans clients. Navajo is spoken. 

Medical services include:

~ Adult, sports and well child physicals ~ Immunizations
~ Acute and chronic illness care ~ Laboratory and radiology services
~ Prenatal care and family planning ~ On-site pharmacy
~ Nutrition and health education services ~ Referrals to specialty care through the
   Indian Health Services or other agencies

    Monday 8 a.m. to 5 p.m.
    Tuesday 8 a.m. to 7 p.m.
    Wednesday 8 a.m. to 5 p.m.
    Thursday 8 a.m. to 7 p.m.
    Friday 8 a.m. to 5 p.m. and

Same day appointments: patients are seen daily. Please call in the mornings for available same day appointments. Same day appointments times are reserved for illness care only. Priority is given to the sickest patients.

Eligibility: Native Americans must present tribal census numbers (CIB) at the time of service. Native Americans without tribal census numbers and non-Native Americans are responsible for whatever fees are not reimbursed by their insurance. Most major insurance plans are accepted. Please call the Family Health Center  to find out. No Native American patients will be denied services.

Health Education Program: Health education topics are presented on an individual or group basis. They include diet and exercise, maternal and child health, dental health, men's and women's health, sexually transmitted diseases including HIV, substance abuse and tobacco prevention, and information on medical conditions and tests. Harmony through Health Diabetes Program: the FHC Diabetes staff provides individualized nutrition assessment, education and case management to people with diabetes. Screenings, blood glucose monitoring, and information on exercise, medications and complications of diabetes are available to individual clients and at community events. Immunizations Program: Vaccinations protect against many infectious diseases. NACA provides free vaccinations to children under age 19 who are established clients. Adult immunizations are available. NACA also offers flu shots on a seasonal basis.
